On arrival at the surgery you will be requested to complete a registration form and you should have the following documents:

  • your birth certificate or NHS medical card, AND
  • a passport or a photo ID driving licence
  • a utility bill, or
  • a note from your employer stating permanent residence

You will also be requested to fill in a New Patient Questionnaire.

You may wish to print off both a registration form and a new patient questionnaire, fill them out and bring them with you on your first visit to the practice. See the links below.
